Brain Aneurysm Screening: Who Should Get Imaging Before Symptoms Start
A cerebral aneurysm is a weak, bulging spot in the wall of a brain artery. Often compared to a thin
bubble on an overinflated tire, an unruptured brain aneurysm frequently produces no symptoms at
all. However, if an aneurysm ruptures, it causes a life-threatening subarachnoid hemorrhage.
Because modern neurovascular techniques can effectively treat aneurysms before they break,
identifying high-risk individuals through non-invasive screening is a critical preventive measure.
Who Should Be Screened for a Brain Aneurysm?
Routine brain aneurysm screening is not recommended for the general population because the overall
incidence of unruptured aneurysms is relatively low (around 3%). However, screening is strongly
recommended for specific high-risk groups:

Strong Family History: Individuals with two or more first-degree relatives (parents, siblings, or children) who have had a documented brain aneurysm or subarachnoid hemorrhage.

Inherited Connective Tissue Disorders: Patients diagnosed with conditions that weaken blood vessel walls, such as Autosomal Dominant Polycystic Kidney Disease (ADPKD), Ehlers-Danlos syndrome (vascular type), or Marfan syndrome.

Coarctation of the Aorta: A congenital narrowing of the main body artery that alters blood flow dynamics and increases intracranial vascular risk.

Aortic Coarctation or Fibromuscular Dysplasia (FMD): Systemic vascular conditions associated with higher rates of intracranial vessel weakness.
How Brain Aneurysm Screening Is Performed
Screening for an unruptured brain aneurysm is quick, non-invasive, and painless:

Magnetic Resonance Angiography (MRA): Uses strong magnetic fields and radio waves to generate detailed 3D pictures of brain blood vessels without exposing the patient to ionizing radiation.

CT Angiography (CTA): Uses a rapid CT scan combined with intravenous contrast dye to visualize arterial structures in high resolution.
What Happens if an Aneurysm Is Discovered?
Discovering an unruptured aneurysm does not automatically mean open brain surgery is required. A
neurosurgical evaluation considers several factors—including the aneurysm’s size, shape,
location, and the patient’s overall health and family history.
Options range from active surveillance with regular imaging to advanced preventive interventions,
such as endovascular coiling, flow-diverter stenting, or surgical clipping.
